When I Die

The Advanced Health Care Directive should be referred to if I become incapacitated (unable to make health decisions for myself):

Basically, I don't want to be on life support. And even if I am NOT on life support, but I'm demented and I don't recognize anyone, will never fully recover from a terminal illness, and I am NOT enjoying myself, then please end my life.

If I am incapacitated and can not make decisions on my own, Lindsey is the sole authority for deciding my living situation and conditions. This authority takes precedent over the decisions to spend my money to make me comfortable. For example, if she decides that I would be most comfortable in a memory care unit at $20,000 / month, then Chris (as the attorney-in-fact) has no choice but to sell stock and make it happen. 

Although Lindsey is listed as the main decision-maker, I would hope that you all talk and arrive at a good decision. June knows my wishes intimately as we have talked about it a lot!

Note: I have filled out the relevant paperwork to donate my body to UCSF.

The original copy of this is held by Chris who becomes the Trustee (executor) when I die.

• First Tech Checking Account (acct# 91609909) is the only account that I have there, and probably has minimal money. Chris is listed as the only designated beneficiary. Jamie is the contingent beneficiary. This is to help defray any costs of dealing with my death (party and whatnot). If anything is left over, distribute it among the siblings.

My Intentions

• In ALL of the accounts / assets, my intent is that everything be divided as equally as possible between my four children. If anyone formally (legally) contests my wishes, they get nothing!
• If any of you with kids pass before me, and I have NOT explicitly made the changes here before I died, then the share that would have gone to you should go to your surviving kids (my grandkids). For example, if Lindsey passes before me, her share would be divided equally between her four girls. I would have stipulated that in the trust, but it gets very complicated. However, I trust all of you will do the right thing when the time comes. 
• As of this writing, I have 'loaned' $30,000 to all of you.

• Anything else that I have that is worth keeping, you guys can fight over it! (just kidding). I would hope that you would all try execute my wishes. Talk about all of the issues and arrive at a wise decision.
• I would also hope that June, if she is still around, would be involved in relevant decisions.
